These Terms of Service govern your use of the VIPCARE.Life website, communications, membership inquiries, and related services offered by VIPCARE.Life, PLLC, a concierge medical practice led by Hemant Gupta, MD. By using this website or requesting information from VIPCARE.Life, you agree to these Terms. If you do not agree, please do not use this website.
VIPCARE.Life is not an emergency medical service. If you are experiencing chest pain, stroke symptoms, severe shortness of breath, major trauma, severe allergic reaction, loss of consciousness, suicidal thoughts, or any potentially life-threatening condition, call 911 or go to the nearest emergency department.
1. Medical Emergency Notice
VIPCARE.Life is not an emergency medical service. This website, membership program, telemedicine access, text messaging, email, telephone communication, and home visits are not substitutes for 911, urgent care, or the emergency department.
If you are experiencing chest pain, stroke symptoms, severe shortness of breath, major trauma, severe allergic reaction, loss of consciousness, suicidal thoughts, or any potentially life-threatening condition, call 911 or go to the nearest emergency department.
2. No Medical Advice Through Website
The information on this website is for general informational purposes only. It does not create a physician-patient relationship and should not be relied upon as personal medical advice, diagnosis, or treatment.
A physician-patient relationship is established only after VIPCARE.Life accepts you as a patient, completes any required intake and consent documents, and confirms that services are appropriate.
3. Concierge Membership Is Not Health Insurance
VIPCARE.Life membership is not health insurance and does not replace health insurance. Membership fees do not cover labs, imaging, medications, specialists, hospitals, emergency departments, urgent care centers, ambulance services, facility fees, outside clinicians, procedures, or services billed by third parties.
Patients remain financially responsible for services not covered by insurance.
VIPCARE.Life does not guarantee insurance coverage, insurance authorization, claim payment, reimbursement, specialist access, hospital admission, medication approval, or any specific clinical outcome.
4. Scope of Services
VIPCARE.Life may provide concierge internal medicine, primary care, preventive care, chronic care support, care coordination, telemedicine, and home visits when clinically appropriate, legally permitted, and logistically feasible.
Services are provided at the discretion of Dr. Gupta and may depend on patient location, medical condition, safety, availability, and applicable law.
VIPCARE.Life may decline, defer, or redirect care if the condition requires emergency evaluation, urgent care, hospital-level resources, specialist care, or a different clinical setting.
5. Telemedicine and State Location Requirement
VIPCARE.Life provides clinical services only when legally permitted. Patients must generally be physically located in Massachusetts or Connecticut at the time of a clinical interaction, unless otherwise permitted by applicable law.
If you are traveling or physically located outside Massachusetts or Connecticut, VIPCARE.Life may be limited to general guidance, care coordination, or referral advice and may not be able to diagnose, treat, prescribe, or provide telemedicine services.
6. Home Visit Policy
Home visits may be available for established members within designated service areas when clinically appropriate and feasible. Home visits are not guaranteed and are not emergency response services.
VIPCARE.Life may decline, reschedule, terminate, or redirect a home visit if:
- The condition may require emergency or hospital-level care
- Adequate privacy is unavailable
- The environment appears unsafe
- Pets are not secured
- Weapons are present or unsecured
- Household members are intoxicated, threatening, or disruptive
- The patient requires services beyond the scope of a home visit
- A chaperone is required but unavailable
- Weather, travel, or logistics make the visit unsafe or impractical
7. Chaperone Policy
Patients may request a chaperone for any examination. Dr. Gupta may require a chaperone for sensitive examinations, including breast, pelvic, genital, rectal, hernia, or other examinations requiring undressing or involving sensitive areas.
If an appropriate chaperone is unavailable, the examination may be deferred, rescheduled, or referred to another clinical setting.
8. Prescriptions
Prescriptions are issued only when medically appropriate, legally permitted, and consistent with clinical judgment, applicable law, and pharmacy requirements.
Membership does not guarantee any medication, refill, antibiotic, opioid, benzodiazepine, stimulant, controlled substance, GLP-1 medication, weight-loss medication, or other prescription.
VIPCARE.Life may require an in-person visit, records review, lab testing, specialist input, or referral before prescribing.
9. Communications
VIPCARE.Life may communicate by phone, text, email, portal, video, or other approved methods. Some communication methods may not be appropriate for urgent issues or sensitive health information.
Messages are not guaranteed to be reviewed immediately. Do not use text, email, voicemail, website forms, or portal messages for emergencies.
Patients are responsible for keeping contact information current and for following emergency instructions when appropriate.
10. Website Forms and Membership Requests
Submitting a website form, email, or consultation request does not establish a physician-patient relationship. VIPCARE.Life may accept or decline membership or care requests at its discretion.
Do not submit urgent medical issues through website forms.
11. No Guarantee of Outcomes
VIPCARE.Life provides evidence-informed, individualized medical care. However, no diagnosis, treatment response, weight-loss result, longevity result, prevention outcome, specialist result, insurance result, medication approval, or health outcome is guaranteed.
12. Patient Responsibilities
Patients agree to provide accurate medical history, medication information, allergy information, insurance information, location information, and contact information.
Patients agree to seek emergency care when appropriate and to follow recommended escalation instructions.
Patients agree not to use VIPCARE.Life services for abusive, threatening, fraudulent, or unlawful purposes.
13. Website Content and Intellectual Property
All website content, branding, text, design, images, logos, and materials are owned by or licensed to VIPCARE.Life unless otherwise stated. You may not copy, reproduce, distribute, or use website content for commercial purposes without written permission.
